A Hazard and Operability (HAZOP) study is a structured and systematic analysis technique for examining a system, with the objective of:
- identifying potential hazards in the system, which may include both those essentially relevant only to the immediate system, and those with a much wider sphere of influence, e.g. environmental;
- identifying potential operability problems with the system and in particular identifying causes of operational disturbances and production deviations likely to lead to nonconforming products.
Failure Modes and Effect Analysis (FMEA) is a systematic procedure for the analysis to identify the potential failure modes, their causes and effects on system performance.
The key feature of a HAZOP/FMEA study is an “examination session”, under the guidance of “Study Leader”, during which a structured multidisciplinary team systematically examines all relevant parts of the system and/or equipment design.
Our role in your HAZOP/FMEA study is that of the “study leader”; a team leader ensuring comprehensive coverage, maintenance of focus, logical thinking and an analytical approach whilst providing the benefits of experience and independent thought.
Timing is essential; if done early enough in the development cycle, then incorporating the design changes to overcome deficiencies identified will, undoubtedly, be cost effective. It is therefore important that the analysis is incorporated into the development plan and schedule.
